MiDAS Booking form - Bristol

To book your place on Bristol Community Transport's MiDAS training course, simply complete this online form. 

Proving your UK licence: 

To attend the course, you'll need to demonstrate you have the correct driving licence and permissions. To do this you'll need to obtain a DVLA Check Code and bring a PRINTED COPY with you on the day of your course. You can get a DVLA Check Code from: https://www.gov.uk/view-driving-licence

  Module 1 - Standard and accessibility. Duration 1 day (course available on Tuesdays)

  Payment should normally be made by cheque after you've submitted your online booking form. Please send your cheque, payable to CT Plus Bristol, to: MiDAS Training, Bristol Community Transport, 137 Parson Street, Bedminster, Bristol BS3 5RB. Alternatively, if you're booking on behalf of an organisation and prefer to be invoiced, you can choose this option, completing the additional information below. Please submit your payment no later than 14 days before your course begins.
